As part of providing you with the highest standard of care, it is necessary for us to collect your personal information including name, date of birth, address, email, telephone number, gender, occupation, medicare, health fund, Veteran Affairs number as well as information about your present and past health, medical conditions, medications and dental history. This information is generally provided by you, but may also be supplied by your referring health practitioner or doctor.

We may disclose or discuss your treatment, item codes, costs of treatment and appointment times with your private health fund and referring health practitioner.

If you require access to your medical records, we will endeavor to provide them promptly and without charge for request, although fees to cover administrative and processing may arise.

De-identified case studies or data may be used for clinical research or educational purposes (including online). If you would prefer not to have your de-identified treatment included for clinical research or educational purposes please notify your clinician.
